Erin Denholm named CMS
Innovation Advisor
Erin is past president
of HCAC and current
chair of the Colorado
Foundation for Home Care
Technology & Education
Erin Denholm, president
and CEO of Centura
Health at Home, has been
named an Innovation Advisor
by the Centers for Medicare
and Medicaid Services
(CMS). Innovation
Advisors will work with
the CMS Innovation
Center to create and
test new models of care
delivery in their own
organizations and
communities, and share
them throughout the
United States. The 73
Innovation Advisors were
selected from 920
applicants through a
competitive process, and
include clinicians,
allied health
professionals, health
administrators and
others. Participating
advisors will deepen
their knowledge in
health care economics
and finance, population
health, systems
analysis, and operations
research. Among other
duties, the advisors
will support the
Innovation Center in
testing new models of
care delivery, forming
partnerships with local
organizations to drive
delivery system reform,
and improving their own
health systems to
achieve better care at a
lower cost for the
communities they serve.

fulfills 1.5 hours
toward Colorado's Home
Care Licensure
Administrator / Agency
Manager required
education. Certificates
of attendance will be
mailed after evaluations
are received.

Fundamentals in Home
Health
This
instructor-led online
learning course has been
updated and revamped and
is now ready for
registrations!
Agencies can complement
their orientation of new
nurses with an online
course available
monthly, either through
a facilitated or
independent study
course. Your nurses will
dive into four dynamic
modules providing them
with the fundamentals
that include planning
and making a home visit
as well as the clinical
applications using
actual case scenarios to
challenge critical
thinking skills.
And, 24 continuing
education credits will
be granted for
coursework upon
completion!
This course has been
written, designed and
developed by home health
nurses.
The
Facilitated Course will
have two weekly
conference calls led by
a home health care
professional, and will
last 4 weeks. The
Independent Study Course
will allow the user to
complete the course at
their own pace.
The
course is expected to
take anywhere between
2-10 weeks to complete
depending on which
format of the course the
student chooses to take.

Homecare Advances in
Management and Practice)
to bring valuable

members. CHAMP is an
innovative web-based
initiative to advance
home care excellence for
older persons. CHAMP’s
website,
www.champ-program.org,
offers a “go to” place
where frontline
clinicians can find
relevant
resources (tools,
practical checklists,
guides, reminders,
patient and clinician
education materials,
etc.) for use in the
home setting. CHAMP also
provides
e-learning courses
tailored to the home
care sector and a
community blog with
advice from colleagues
on key topics for
geriatric care. In
addition, there is a
growing inventory of
evidence-based tools to
help providers trying to
make a real difference
in the lives of older
people who receive care
at home.
