ACH Campaign:
CFMC & HCAC As local Area Networks of Excellence (LANES), HCAC and CFMC are registered with CMS to assist registered agency participants in the HHQI National Campaign to reduce Acute Care Hospitalization among their patients. LANE roles are to facilitate information and intervention transfer from CMS, interpretation of data reports, overcoming barriers to progress in ACH rate reduction, and disseminate success stories and strategies.
